The rational and appropriate indication of medicines prescriptions to every patient is a necessity so that the optimal therapy can be established. In fact, there is prescribed medicine that inappropriate with the official information of medicines and the circulation license called as off-label medicines. The medicines which is categorized as off-label indication is when the medicines which is listed in the leaflet. This research aims to know indication prescribing of off-label indication of medicines prescription on hospitalized adult patients in PKU Muhammadiyah Yogyakarta Hospital. This research included in observational descriptive design in cross-sectional method. The data collecting of off-label prescription had been done retrospectively. The technique used in it was systematic random sampling in which the data were taken from the medical records of hospitalized adult patients during the period of January to December 2014. The data processing had been done using descriptive analysis. The research was done in August 2015 until May 2016. The research resulted from total sample 354 medical record data patients that appropriate with inclusion criteria. Based on 1.306 medicines prescriptions, there were 4 (0,22%) which include in off-label indication category medicines. The list of off-label which prescribed during January to December 2014 was Misoprostol, Ondansentron, and Domperidone.
