Indonesia is a country with the high biodiversity. About 940 species have been declared as herbal plant. Traditional medicine has been used widely as an alternative treatment. The usage of traditional medicine was based on perception that traditional medicine is safer than modern medicine. Traditional medicine will be beneficial and safe if used properly, the dose, timing, and method of use, selection of materials and indication. This research aims to determine the level of knowledge and to describe the usage of traditional medicine as an alternative treatment in RW 005 Desa Sindurjan, Kecamatan Purworejo, Kabupaten Purworejo. The type of this research is descriptive non-experimental. 171 respondents were participated in this research. Samples were selected by purposive sampling. The level of knowledge was measured using questionnaire and assessed by the Guttman scale (statement true or false). The data were analyzed statistically and descriptive. The level of knowledge were categorized into good, sufficient and less. The result, the level of knowledge in RW 005 Desa Sindurjan is sufficient (56%). Traditional medicine was used because it’s easy to found (44%), the source information based on experience (38%), traditional medicine was used to treat mild disease (50.9%), the dosage form mostly used is decoction (65.5%), the using of traditional medicine until cured (58.5%), traditional medicine did not bring adverse effect (98.8%), total of 126 respondents (73.7% ) do not know the name or content of traditional medicines which they consumed, traditional medicine mostly used are kencur (3.5%) and ginger (3.5%), and total of 169 respondents (98.8%) feel the effect of recovery after consumed traditional medicine
